A full continuum of care is necessary for successful, long-term recovery. Clear communication channels between providers and the recovery center ensure clients are supported throughout the substance abuse treatment process.
In order to establish efficient communication standards upfront, our admissions team collects each client’s medical records upon their admission into treatment. By fully understanding each client’s medical background, the clinical team can work seamlessly with their previous providers as individuals transition into treatment.
Upon obtaining a Release of Information agreement, the clinical team can provide updates on the client based on their provider’s communication preferences.
The providers and recovery center can collaborate on their shared client’s treatment plan and ensure everyone’s goals are aligned. As a result, when clients graduate treatment and begin their after-care, they can continue progressing in their recovery journeys.
Efficient communication channels allow for clients to receive comprehensive treatment, that not only benefits their stay in residential treatment but also ensures that there is a clear, supported plan for their long-term care.Â
